To make an appointment, please call 718-230-1180. We will need your doctor's name, your diagnosis and your insurance information. A physician referral is not usually necessary, but in certain cases the insurance company may require one. Our office staff will call the insurance company to verify your coverage. If your insurance company requires authorization, the office will let you know.
